This agreement will remain in effect until further notice, and the client agrees to all the terms listed below.Consent* I certify that my pet has not harmed or shown aggressive or threatening behavior toward any other person or any other dog and that my dog(s) is in good health.Consent* I agree that I have notified Hilltop Dog Hotel of any and all health conditions, abnormal behaviors, or issues that would be pertinent to the care of my dog(s).Consent* I agree to pay for special fees and daily boarding at the Hilltop Dog Hotel’s posted rates. If my dog requires special care, I authorize Hilltop Dog Hotel to provide that care and agree to pay for those services.Consent* I agree to pay for any additional services as requested by me such as playtime, nature walks, bathing, grooming, etc.Consent* I understand that while Hilltop Dog Hotel associates will always be present to monitor playtime, injuries can and do occur due to vigorous play and exercise. I agree that I am solely responsible for any incidental injury that may occur to my dog(s) and I also agree to be responsible for any resulting veterinarian charges. I further understand that Hilltop Dog Hotel will do its best to monitor my dog(s) intake of food and water. If Hilltop Dog Hotel notices my dog(s) intake of food may not be sufficient, I understand I will be notified if this becomes a serious concern.Consent* I understand that I am solely responsible for any harm caused by my dog(s) while my dogs(s) are at Hilltop Dog Hotel and they will not be held liable for injuries resulting from abnormal behavior (failure to eat, drink, eating debris such as rocks, feces, bedding, or toys, allergic reactions, etc.). I agree that I am solely responsible for any damages that the above-mentioned dog(s) may cause through malicious or improper conduct and that Hilltop Dog Hotel is not responsible for the loss or damage of any personal items left at the facility or with the dog(s).Consent* I authorize Hilltop Dog Hotel to do whatever is deemed necessary for the health and well-being of my dog(s) and agree to pay for any and all expenses relating to the same. If the dog(s) becomes seriously ill, the owner shall be notified at once. If a dog, which is presented for boarding, grooming, or playtime is found to have fleas and/or ticks, it will be treated and/or bathed at owner’s expense. Hilltop Dog Hotel strongly recommends each pet be on flea and tick prevention as they can carry diseases.Consent* I understand that my pet can become ill as the result of having contact with other pets, similar to children in daycare. Hilltop dog Hotel will not be held liable for any illness that my pet contracts while staying at the facility.Consent* I understand that Sunday pick-up is from 4pm to 6pm only that the fee for this service is $30 + tax. All boarding charges must be pre-paid upon drop off.Consent* I understand that Hilltop Dog Hotel has the right to rehome any pet that is not picked up within 5 days passed the expected reservation date, following 3 failed attempts to contact the rightful owner and the provided emergency contact.Hilltop Dog Hotel would like permission to photograph, video and/or digitally record, and/or reproduce any other manner of the image and/or likeness and activities of my dog(s), for unlimited use of such image and/or likeness, on its website, in brochures, in other advertising and/or marketing materials, or for any other purpose. By agreeing, I understand that I will not receive any compensation for any use by Hilltop Dog Hotel.
